Immunizations Needed for School
As of Sept. 1, 2014, Minnesota's Immunization Law requires students who are entering early childhood education programs, Kindergarten, 7th grade, and all newly enrolled students to be in compliance with the Minnesota State Immunization Requirements. Medical and conscientious exemptions are allowed under the law.

Student Immunization Form - Early Childhood
Attach a copy of your child’s immunizations record from their health care provider to this form. Use this form to claim an exemption from immunization requirements for medical reasons or conscientiously held beliefs of the parent or guardian. For exemptions due to conscientiously held beliefs, it is necessary to provide a notarized exemption form to the health office.
No Shots, No School!
In order to attend school, students must show they're in compliance with Minnesota State Immunization Requirements (see Minnesota Department of Health for more).
It is recommended that you call your child's physician to schedule immunization appointments. Bloomington Public Health offers low cost immunization clinics. Dates and times of community immunization clinics can be obtained by calling Bloomington Public Health at (952) 563-8900. Clinic hours are Monday-Friday 8:00 am - 4:30 pm.
